• Parents and students, the Code of Student Conduct Handbook Goes Green!

     Welcome to the 2020/21 school year. Emergency Contact and Code of Student Conduct forms for your child can be completed online. Forms are easy to complete and all data is protected and secure. For families with multiple children attending Broward County Public Schools, some information will only need to be entered one time.

    The online forms can be accessed below via the Back to School wizard, which walks you through the process. You will need your child’s student identification number to complete the forms, which are provided in English, Spanish, Haitian Creole and Portuguese.

    The Emergency Contact and Code of Student Conduct forms must be completed at the start of each school year for each student enrolled in the District. It is recommended that you set aside 20 to 30 minutes for this process. If you are not able to complete the online forms in one session, you can resume where you left off at another time. Once you complete and submit the forms, you will receive an email confirmation. 

    If you prefer to use hardcopy forms you can download them at Code of Conduct Information. The completed forms must be submitted to your child's school.
