    Welcome to Athletic Forms

    In order to be eligible to try out for a team or participate in any pre-season training, each student must have the following forms completed and filed in the Athletic Office no less than one day prior to the first day of tryouts or training.  A delay in submitting the completed forms will prevent a student from participating, thus missing tryouts and possibly not making the team.

    Students must complete the necessary forms below in order to try out or practice with their teams.

    1. STUDENT ATHLETE REQUIRED DOCUMENTATION CHECKLIST 
    1. PHYSICAL EVALUATION (FHSAA Form EL2 – Pre-participation Physical Examination) 

    Students must have a physical evaluation each year and be certified a Physical Evaluation Each Year. A student must have a physical evaluation each year and be certified as being physically fit to participate in interscholastic athletic programs. The student cannot be allowed to participate in any activity related to interscholastic athletic programs until the fully executed physical evaluation form is on file in the school. (s.1002.20(17)(b), Florida Statutes)

    1. CONSENT AND RELEASE (FHSAA Form EL3 – Consent and Release from Liability Certificate)

    Students must have the consent of his/her parent(s) or legal guardian(s) to participate in interscholastic athletic programs at Hollywood Hills High School.  The student and his/her parent(s) or legal guardian(s) must also release Hollywood Hills High School and the FHSAA and contest officials from all liability for any injury or claim that may result from the student’s participation in interscholastic athletics.  The form must be signed by the student and his/her parent(s) or legal guardian(s). The student cannot be allowed to participate in any activity related to interscholastic athletic programs until the fully executed consent form is on file in the school.

    1. CONCUSSION AND HEAT RELATED ILLNESSES RELEASE FORM (FHSAA Form EL3CH)

    The form must be signed by the student and his/her parent(s) or legal guardian(s). The student cannot be allowed to participate in any activity related to interscholastic athletic programs until the fully executed consent form is on file in the school.

    1. PARENTAL CONSENT FOR TREATMENT

    This form is designed for those situations where minors are unaccompanied by either parents or legal guardians.  This “Medical Treatment Authorization and Consent Form” gives authority to arrange for medical care for a minor in the event of an emergency. This is extremely important, in that, medical care can not be provided to a minor without approval by the parents or legal guardians, unless there is written consent authorizing an agent to give approval.

    1. MEMORANDUM OF UNDERSTANDING TO PLAY SPORTS AT HOLLYWOOD HILLS HIGH SCHOOL (YEARLY)

    This is an agreement between Hollywood Hills High School student athletes and Hollywood Hills High School.  The Memorandum of Understanding (MOU) sets forth the responsibilities of student athletes with a view to promoting good character, enhancing the integrity of education at Hollywood Hills High School, and promoting civility in society and the sports arena.
