    2019-2020 Head Start Registration Information

      *You Must Register Your Child By July 19th* 
    Early Head Start ~ Please check your email to complete your school registration forms

    If you received an email or letter stating your child's Enrollment Status is: Accepted into the Head Start program for the 2019-2020 school year, please do the following:

    Step 1. Complete the School Registration Forms online  **Click here for Spanish, Haitian-Creole, and Portuguese

    Step 2. Go to the school where your child was Accepted and completed the registration process

    Step 3. Bring ALL required registration documents below to the school  

    *You must register your child by July 19th*

    Required Health Documents

    • Original (up-to-date) Florida Certificate of Immunization (DH Form 680)
    • Physical Exam - Must include the following:
      • Lead Test Results
      • Anemia Blood Testing (Hct or Hgb)
      • Height & Weight
      • Blood Pressure
    • Proof of Dental Exam (new requirement) Dental Form

    School Registration Documents

    • Proof of Age (child’s ORIGINAL birth certificate or passport)
    • Parent/Guardian photo identification
    • Proof of Broward County Residence (domicile)
      If you OWN or RENT your residence submit one document from both Columns A and B. All documents must be current, valid, and include the residential address used for enrollment.
      • Property tax bill
      • Homestead exemption card
      • Deed
      • Mortgage statement
      • Home purchase contract
      • Notarized lease agreement
      • Utility bill (i.e., electric, water, waste)
      • Telephone or cellular phone bill
      • Verification of Tenancy letter from the homeowners or condominium association
      • Declaration of Domicile Form from the County Records Department
      • Florida drivers license
      • Florida identification card
      • Automobile registration
      • Automobile insurance
      • Credit card statement
      • Two consecutive bank account statements
      • U.S. Postal Service confirmation of address change request
    • If you SHARE the housing of another person who owns/rents the home:

      • Both the registering parent and owner/renter of the residence complete a notarized Affidavit of Shared Residence Form:
        English | Spanish | Haitian/Creole | Portuguese; and,
      • The owner/renter of the residence must submit one document from both Columns A and B; and,
      • The registering parent must submit two documents from Column B.

    Early Head Start will not participate in the Enrollment Round-up. Please check your email to complete your school registration forms.
