    2019-2020 VPK/Enrichment Program Registration Information

    If you received an email or letter stating your child's Enrollment Status is: Selected – Ready for Registration/Enrollment at the School into the VPK/Enrichment program for the 2019-2020 school year, please do the following:

    Step 1. Print the Selected – Ready for Registration/Enrollment at the School letter or email

    Step 2. Gather needed documents (see below) and your printed email or letter

    Step 3. Visit your assigned school, accept the seat and complete the registration forms, OR you can complete the VPK School Registration Forms online then print and bring them to the school **Click here for SpanishHaitian-Creole, and Portuguese


    If you received an email or letter stating your child’s Enrollment Status is: Eligible & Selected – More Information Neededplease do the following:

    Step 1. Do not visit the school

    Step 2. Call the VPK office at 754-323-4709 or email vpk@browardschools.com to determine the information that is needed

    Step 3. Submit the requested information as per instructions given by the VPK Office

    Documents needed for School Registration:

    • Proof of Age (child’s ORIGINAL birth certificate or passport)
    • Parent/Guardian photo identification
    • Proof of Guardianship (if applicable)
    • Proof of Broward County Residence (domicile)

    Required Health Documents

    • Original (up-to-date) Florida Certificate of Immunization (DH Form 680)
    • Physical Exam (within the last year)

    School Registration Documents

    • Proof of Age (child’s ORIGINAL birth certificate or passport)
    • Parent/Guardian photo identification
    • Proof of Broward County Residence (domicile)

      If you OWN or RENT your residence:

      • Submit one document from both Columns A and B

      If you SHARE the housing of another person who owns/rents the home:

      • Both the registering parent and owner/renter of the residence complete a notarized Affidavit of Shared Residence Form:
        English | Spanish | Haitian/Creole | Portuguese; and,
      • The owner/renter of the residence must submit one document from both Columns A and B; and,
      • The registering parent must submit two documents from Column B.

      If you answer “yes” to certain residency questions on the Student Registration Form you may qualify for the HOMELESS Education Program (HEP) under the McKinney-Vento Act.

      • The school will provide a referral to the district’s Homeless Education Liaison and, if qualified, the student will be eligible for immediate services.
      • Students registered under the McKinney-Vento Act must re-enroll each school year.

      • Declaration of Domicile Form from the County Records Department

      • Automobile registration
      • Automobile insurance
      • Credit card statement
      • Two consecutive bank account statements
      • U.S. Postal Service confirmation of address change request
