    2020-2021 Head Start/Early Head Start Registration Process

    If you received an email stating your child was selected into the Head Start/Early Head Start program for the 2020-2021 school year, please do the following:

    Step 1. Complete the Head Start/Early Head Start School Registration Form online. Once you complete and sign the form, you will receive a copy in your email. Please upload your signed copy into the parent portal  *English|SpanishHaitian/CreolePortuguese

    Step 2. Upload a copy of the mandatory documents below into the Head Start parent portal

    Step 3. Please contact your child's assigned school to complete the registration process

    Mandatory Health and Registration Documents:

    1. Original (up-to-date) Florida Certificate of Immunization (DH Form 680)
    2. Physical Exam Form (within last year). This includes:
      • Lead Test Results
      • Anemia Blood Testing (Hct or Hgb)
      • Height & Weight
      • Blood Pressure
      • Vision & Hearing
    3. Proof of Dental Exam (New Requirement). Click to complete the Dental Form: (Dental Form)
    4. Proof of Age (child’s ORIGINAL birth certificate or passport)
    5. Parent/Guardian photo identification
    6. Proof of Guardianship (if applicable)
    7. Proof of Broward County Residence (Please see list of acceptable proof of residence documents below) 

    Note: If you share the housing of another person who owns/rents the home, both the registering parent and owner/renter of the residence complete a notarized Affidavit of Shared Residence Form English|Spanish|Haitian/Creole|PortugueseIf you OWN or RENT your residence: Submit one document from both columns A and B: 

    • Property tax bill
    • Homestead exemption card
    • Deed
    • Mortgage statement
    • Home purchase contract
    • Notarized lease agreement
    • Utility bill (i.e., electric, water, waste)
    • Telephone or cellular phone bill
    • Verification of Tenancy letter from the homeowners or condominium association
    • Declaration of Domicile Form from the County Records Department
    • Florida drivers license
    • Florida identification card
    • Automobile registration
    • Automobile insurance
    • Credit card statement
    • Two consecutive bank account statements
    • U.S. Postal Service confirmation of address change request

    If you answer “yes” to certain residency questions on the Student Registration Form you may qualify for the HOMELESS Education Program under the McKinney-Vento Act.

    • The school will provide a referral to the district’s Homeless Education Liaison and, if qualified, the student will be eligible for immediate services.
    • Students registered under the McKinney-Vento Act must re-enroll each school year.
