Hazel Health

Hazel Health offers all public-school students access to high-quality telehealth services. Any school staff member can initiate a video visit with a Hazel healthcare provider, as long as the parent/guardian has provided consent both on the back of the Student Emergency Contact Card (SECC) and Hazel Health forms. These services are free for families, regardless of insurance status, and help reduce absenteeism, increase instructional time, and improve academic performance. Hazel Health also supports care at home and minimizes lost work time for parents/guardians.

Hazel Health Telehealth Services 

Hazel Health telehealth services are available for students with parent or guardian consent, at no cost to families, and can be accessed from home or school using any tablet, smartphone, or computer with WiFi. Parents can participate in their child’s visits virtually and receive care coordination, including prescriptions or referrals to local providers. Medical care is available Monday through Friday, 7:00 am – 5:00 pm.

How to Enroll in Hazel Health Telehealth Services

  • Complete the Consent Form:
    Parents or guardians must fill out a Hazel Health consent form for each child. This form is required before any telehealth services can be provided.
  • Ways to Access the Consent Form:
    • Online: Visit https://my.hazel.co/broward to complete the consent form electronically.
    • Paper Form: Request a paper consent form from your child’s teacher or the school’s front office. Return the completed form to the school.
  • No Cost to Families:
    Hazel Health services are provided at no cost to families, thanks to the partnership with Broward County Public Schools.
  • After Enrollment:
    Once the consent form is submitted, your child can access Hazel Health telehealth services from home or school using any tablet, smartphone, or computer with WiFi. Parents can participate in visits virtually and receive care coordination, including prescriptions or referrals.
