The following information is to assist you, as the parent/guardian, with providing health information required for your child by Broward County Public Schools. If you have any questions, please do not hesitate to contact your school.

• Print Parent Letter

The Medication Authorization Form allows school staff to administer prescription or over‑the‑counter medications to a student during the school day or school‑sponsored activities. Written authorization from both the parent/guardian and an authorized prescriber is required and must be renewed every 12 months or when changes occur. Parents deliver and sign in medications, which must be in original, properly labeled containers. Medications are administered by healthcare staff or trained designees. Emergency self‑carry medications require specific approval. Unused or expired medications must be collected by parents or will be destroyed after notification attempts.

• Authorization for Medication/Treatment Form

This form authorizes students to self‑carry and self‑administer approved over‑the‑counter topical products, such as sunscreen and insect repellent, with parental permission. Parents accept full responsibility, confirm age appropriateness, and acknowledge that products must be properly labeled, sealed, and used according to manufacturer instructions.

• Authorization for Over-the-Counter with Parental Approval Form (Grades 9-12 only)

This form authorizes a student to carry and self‑administer FDA‑approved over‑the‑counter headache medication at school. Parent and student acknowledge responsibility, proper use, and non‑sharing of medication. The school assumes no liability, and privileges may be revoked if misuse or safety concerns occur.

This form allows students to self-carry and self-administer approved OTC topical products, such as sunscreen and insect repellent, with parental consent. Parents accept full responsibility, confirm products are age‑appropriate, properly labeled, sealed, and used according to manufacturer directions.

• Authorization for Over-the-Counter (OTC) Topical Products with Parent Approval Only

This form documents authorization for a qualified student’s use of medical marijuana at school. Parents and caregivers assume full responsibility, as school staff do not administer the medication. Required information includes physician and caregiver details, dosage instructions, and acknowledgment of liability and disciplinary consequences for misuse

The consent for School Health Services can be found on the back of the Emergency Contact Form. The form should be completed, signed, and dated by the parent/guardian to ensure students can receive health services at school or in school-sponsored activities.

Make sure your child’s required immunizations are up to date. If you are not sure, you can check with your healthcare provider or the Florida Department of Health-Broward at (954) 467-4700 or visit https://flshotsusers.com/patients-and-parents/request-your-immunization-records.

Parents may obtain medical exemptions from their healthcare provider or a religious exemption from the Florida Department of Health Broward.

The Diabetes Medical Management Plan outlines required diabetes care for a student at school. It details blood glucose monitoring, insulin administration, emergency treatment for high and low blood sugar, self‑care permissions, and staff responsibilities, with signed authorization from parents and healthcare providers

Health conditions are defined as those conditions that last 1 year or more and require ongoing medical attention, limit activities of daily living, or both.

All students entering Broward County Public Schools for the first time must have a medical examination performed within one year of registration. The medical examination should be documented on the Florida Department of Health Form 3040 or the provider’s office/medical facility stationery. The appropriate form/stationary should be completed, signed, and dated by the healthcare provider.
