Registration Information

STEP 1: COMPLETE Required Registration Documents

• Upload copies of the REQUIRED Health and Family Enrollment Forms and Medical Records to Head Start using your individual document upload link
• If you do not have a document upload link, please email copies of your documents to our office OR contact 754-321-1961 Option # 4 to request the link

• Original Florida Certificate of Immunization (DH Form 680)
• Proof of Dental Exam. Click here for the Dental Form
• Physical Exam Form (within last year), this includes:
  • Lead Test Results
  • Anemia Blood Testing (Hct or Hgb)
  • Height & Weight
  • Blood Pressure
  • Vision & Hearing

STEP 2: RegistER YOUR CHILD

Contact your child's assigned school to schedule an appointment to complete the registration process. 

List of REQUIRED ORIGINAL documents to bring to register your child at school:

• Proof of Age (child’s ORIGINAL birth certificate or passport) 
• Parent/Guardian photo identification 
• Proof of Guardianship (if applicable) 
• ALL ORIGINAL enrollment forms and medical records listed above in Step 1
• Proof of Broward County Residence (Please contact the school for acceptable proof of residence) 

Note: If you share housing with another person who owns/rents the home, both the registering parent and owner/renter of the residence need to complete a notarized Affidavit of Shared Housing Form | English | Spanish | Creole | Portuguese |
