Applications

1.    We verify that the information provided on the scholarship application matches with the information on the Summer Camp application, the Household Member Form, Scholarship Application, and the Addresses & Contact panel in FOCUS.

2.     All students listed on the application must have a 10-digit student number. If you don’t know it or don’t have it, you can check your Parent FOCUS account or you can contact the school’s front office so they can provide you with the number.

3.     You must provide an active, monitored email address;

• Updates on the status of your application will be sent to the email address you have provided.
• Check your email’s spam, trash, and junk folders for scholarship status emails.
• It is important to monitor your emails for updates on your scholarship application.
  • If you don’t have an email address, free email addresses are available from Yahoo, Gmail, etc.

4.     Submit additional applications if you have more than three children or they attend different schools.

5.    All students listed on the application must have a completed application and have all applicable program fees paid in full.

6.    Names and addresses for Parent/Guardians must match on all the following documents:

• Scholarship application
• Summer Camp application form
• FOCUS Addresses & Contact panel - Contact Information
• Household Member form

7.     To determine eligibility, we will be using total household occupants and gross income for all adults 18 years or older from the Household Member Form.

Before starting the scholarship application, make sure you have reviewed the checklist. Failure to have all the items listed above will delay the application process and/or cause your application to be denied.

Please read these guidelines completely and have all documents ready to attach to the application.

All adults in the household, 18 years or older, are required to submit:

• Current work verification. 
  • Two most current paychecks showing a minimum of 30 hours worked per work week
    • Must submit two most recent paycheck stubs for all adults in the household
    • All adults in the household are required to work a minimum of 30 hours per work week during summer camp operations if approved.

• Most current year (2025) tax return.
  • For acceptable tax documents, view Parent Guide.
• Any related schedules (if applicable).
  • Class Schedule that indicates school name, your name, class name, class location, start/end time for each class, start/end dates for each class, and paid status. On-line classes are not eligible.

• If self-employed:
  • Parent will fill out Self-employment form attached to the application and submit requested income documentation.
