Registration Information
Once students are accepted through the magnet office the following items are needed for registration if you are coming from a Private, a Charter or Out of County Public School click here
COMING FROM PRIVATE SCHOOLS OR OUT OF COUNTY PUBLIC SCHOOLS
**Please bring the following items to registration:
1. School Registration Form (click here to complete and print)
2. Record Request Form (click here to complete and print)
3. Copy of most recent report card (First Semester)
4. Birth Certificate (copy)
5. Blue or White 680 Health Immunization Form (original)
6. Physical Form (original dated within the year)
7. Two (2) Proofs of Address (one from Column A and one from Column B)Column A

- Current telephone or electric bill in the name of the person registering the child
- Mortgage statement
- Notarized lease agreement with name, address and phone number of leaser
- Mortgage commitment
- Home purchase contract including specified closing date with copy of deed to be provided within 60 days of closing date
- Homestead exemption card
- Automobile insurance
- Current Florida Driver's License/Florida
- Identification card
- Cellular telephone bill
- Credit card statement
- Bank account statement
- United States Postal Service confirmation of address change request
- Declaration of Domicile form from the County Records Department
* SCHEDULES WILL NOT BE FINALIZED UNTIL RECORDS ARE RECEIVED FROM THE PREVIOUS SCHOOLTranscripts
FOR CURRENT STUDENTS AND GRADUATES AFTER 2005 Students may pick up the Transcript Request Form in the School Counseling Office or click here to obtain the request form . Once completed, bring the form and $2.00 (per transcript) to the Registrar’s Office in the School Counseling Department or mail to: Pompano Beach High School Attn: Registrar 600 NE 13th Avenue Pompano Beach, FL 33060 Please allow 1-3 days for processing. Fee Hard copy transcripts are $2.00 each. Electronic transmissions (Florida State Public Universities) are free of charge. SAT scores or ACT scores need to be sent directly from www.collegeboard.org or www.act.org to the universities you are applying to as they are not included on your transcript.
