There are many forms that need to be completed each year for all students. All required (and optional) forms are provided in the first day packet, or whenever new students register. These pdf versions are made available as a convenience in the event the original form has been lost, misplaced, or if changes are being requested. This form acknowledges that both the student and parent have read the Code of Student Conduct as well as reviewed the responsibilities listed on the form. On the link above, click where it says "Signature Forms".
This form has two sections. Please note that by checking "I will not" in Section B, your child's picture cannot appear in his / her class picture, school yearbook, or Panther Paws (in addition to those places listed for that section).
IMPORTANT: If this form is not returned, or if either section is left unchecked, the school will interpret this as permission IS given.
Be sure to check ONLY those items you DO NOT wish to have disclosed. NOTE: Checking "Student's Name" may prevent the student's photograph from appearing in the school's yearbook.

Parkside Elementary is a Title I school. This compact is an agreement regarding the partnership in providing appropriate Title I services to our students.
All students participate in a Science Lab special during each quarter. This contract insures that students understand their responsibilities when participating in this program.
This form only needs to be completed and returned if you DO NOT want your child to participate in any of the Family Life / Human Sexuality lessons.
Please refer to the information contained in the Code of Student Conduct Summary of Changes booklet, or contact your child's teacher, if you have any questions related to the curriculum content for your child's grade.
