Flag Football Tryouts for Boys' and Girls'
You must register in-order to attend try-outs. A completed 2021 Athletic packet MUST be turned in BEFORE you can try-out. A COMPLETED packet includes 1. The Insurance Form completed and notarized with a copy of your health insurance card 2. A completed health form completed by a doctor and stamped by the doctor. 3. A completed Assumption of Risk, Waiver, Release & Hold Harmless form

Immunization Requirements for Students Entering the 7th Grade (2020-2021)
All students must provide a Florida Certificate of Immunizations (DHH 680) form stating that their immunizations are complete for 7th-grade entry. Please note that your child needs to be in compliance with the Health Department of the State of Florida immunization requirements. Children entering the 7th grade in Florida schools MUST have received one dose of the tetanus-diphtheria-pertussis (TDAP) vaccine. Contact your health care provider or call the Florida Department of Health in Broward County at (954)-467-4705 for an appointment.
