New Miramar Student Registration

Thank you for choosing to enroll your little one at Miramar Elementary. These are just a few notes to help with the registration process. Attached you will find a checklist regarding which documents are required to enroll your child. Please note that for proof of residence, evidence such as a bill must be dated within the last 30 days. If you do not share housing with another person/renter, please disregard the Shared Housing Form. Please note that the forms are all writeable as well just make sure to complete every section of the registration to make the process easier.
If your child will be enrolling in PreK, please be advised that up to date medical records (physical, shot record, dental) are required. 
Once completed, you have several options to turn in the registration forms and support documentation (noted below).  Kindly note that registration documents do not have to be turned in until mid-July.
