Updated Student Registration Form

The Student Registration Form was updated to reflect four additional questions under “Has the student previously been” section: 

  • Assessed for a behavioral threat?
  • Has an active monitoring plan?
  • Assessed for risk of suicide or self-harm?
  • Has an active safety plan?
