Due to COVID.  YOU MUST TRYOUT THE DAY OF YOUR GRADE LEVEL TRYOUT.

YOU MUST have all these forms completed to tryout.  

1. Covid release form signed
2. Parental permission form signed and Notarized
3. Physical by thier child's Pediatrician or any Broward Health Urgent care facilities must be signed by the Dr.
4. A copy of health insurance card
5. MUST have a 2.0 GPA from the 2nd qt.
