As part of the enrollment requirements for grades 7 and 8 to enter school, and in accordance with Florida Statute 232.032, your child will need to have the following immunizations on file before their first day of school:

• Hepatitis B series (3 doses)
• Measles Vaccine 2nd dose (preferably MMR)
• Tetanus-Diphtheria Booster (TDAP). This booster is required before students enter 7th grade by the State of Florida.

As your child receives his/her updated immunizations, please email the original Florida 680 form to yamely.franco@browardschools.com. We will be accepting updated forms throughout the year.
