Parents can submit Back-To-School forms on-line in your language of choice. The on-line forms wizard replaces the paper "Emergency Contact Card" and "Code of Conduct" forms. Just complete the wizard entirely and print the confirmation page along with any required medical forms, sign and return them to your child's 1st period teacher.
