BCPS Parent Permission Refusal Form for School Climate Survey

In an effort to support school safety and security, inform policy, and provide greater intervention and resources to schools, the School Climate & Discipline Department is coordinating a districtwide School Climate Survey. Students will take the survey will be taken on February 3 & 4 in the classroom. If parents would like to opt-out, please complete the Opt-Out form and return it to Ms. Incarnacao in 2418.
