Immunization Information

About Immunizations

Before attending school in Florida (kindergarten through 12th grade), each child must provide a Florida Certification of Immunization (DH 680 form), documenting their vaccinations.  For further information regarding vaccinations requirements, parents are urged to visit the Broward County Public Schools' Coordinated Students Health Services Office or click on the preferred link below:

Private healthcare providers may grant a Temporary Medical Exemption (TME), documented on the Form DH 680, Florida Certification of Immunization, for those who are in the process of completing any necessary immunizations. The TME requires an expiration date after which the exemption is no longer valid, and the immunizations must be completed before or at that time.  For more information regarding exceptions from required immunizations, click HERE.
