All schools must obtain parent/guardian consent for school health services. All parents must provide written consent for their child to receive school health services while in school. The letters below are distributed to all parents/guardians at the beginning of the school year.

Students will not be able to receive health services in the school clinic without a Parental/Guardian Consent for School Health Services form on file, signed by the parent/guardian.
