If you are ready to register your child for the next school year, we are happy to schedule an appointment with you.

The following documents are required for registration:

- ORIGINAL BIRTH CERTIFICATE OR PASSPORT (Child must be 5 years old on or before September 1, 2020)
- CURRENT IMMUNIZATION ON HRS FORM (SHOT RECORD)
- PHYSICAL EXAM DATED WITHIN ONE YEAR OF THE DATE THE CHILD ENTERS SCHOOL
- TWO (2) PROOFS OF RESIDENCY - MUST BE MORTGAGE OR LEASE UP-TO-DATE AND ONE BILL TO THAT ADDRESS (FPL,  PHONE ETC...)
- PARENT/GUARDIAN PHOTO I.D. WITH CURRENT ADDRESS
