Pfizer Fact Sheet (Ages 5-11)

The Florida Department of Health, in partnership with Broward County Public Schools, is providing voluntary free Pfizer COVID-19 vaccinations at Heron Heights this Wednesday, November 10th. The vaccine is optional and requires parental consent.  Paper copies of the consent form and important information about the approved vaccine are coming home with your child today.

Your child will need the consent form completely filled out to receive the vaccine at school.  Parents are invited to be present with their child for the vaccine.  If you would like to be present, DO NOT send the paper form in with your child; please BRING the form to the school between the hours of 11:00am-1:30pm on WEDNESDAY and we will call your child out of class to receive the vaccine.  You may also choose to return to school with your child between the hours of 2:30pm-6:00pm for the child to receive the vaccine.

Students may get the vaccine at school without a parent present as well.  If you would like for your child to receive the vaccine during school hours without a parent present, complete the consent form in its entirety and send the paper copy back to school tomorrow or Wednesday at the latest.  Even if your consent form is received, if your child refuses to get the vaccine and a parent is not here at school the Department of Health will NOT administer the vaccine. 

The second dose of the vaccine will be available at school on December 1st.  An additional consent form will come home closer to that date.

We will also be offering the vaccine on January 19th with the second dose being administered on February 9th.
