Dear Parents,

We are receiving registrations by email or hard copy. If you choose to send the registration package by email, please fill out the attached forms, save them to your desktop and send them to the e-mail address below as a PDF file (no photos please) or if you choose to bring a hard copy of the documents, print them out and drop them off at the main office. When you are ready to register your child, be sure to review the Information for Student Registration checklist before you arrive. Other than that, send:

• Mortgage statement / warranty deed, or lease contract
• Sharing housing form (when applicable) (Must be notarized)
• Utility bill
• Copy of driver's license
• Last report card (when applicable)
• Birth certificate
• Physical within last year, updated Immunization 680 form (from a Dr. in Florida)

All of them in a PDF file (no photos please).
