• Dear Broward County Public Schools Parents and Guardians: 

    Welcome to the 2020/21 school year. This year, the Emergency Contact and Code of Student Conduct forms for your child can be completed online. Online forms are easy to complete and all data is protected and secure. For families with multiple children attending Broward County Public Schools, some information will only need to be entered one time.  

    Visit School Registration to access the Back to School wizard, which walks you through the process of completing the forms.  Please note, you will need your child’s student identification number to complete the forms, which are provided in English, Spanish, Haitian Creole and Portuguese. 

    The Emergency Contact and Code of Student Conduct forms must be completed at the start of each school year for each student enrolled in BCPS. It is recommended that you set aside 20 to 30 minutes for this process. If you are not able to complete the online forms in one session, you can resume where you left off at another time. Once you complete and submit the forms, you MUST print and submit the signed confirmation to your child’s school. You will receive an email confirmation indicating that you successfully submitted the required forms.
