Registration and Health Services Overview

  • New Student Registration
    School registration for new students is in person at our school. Registration is important and we want to make sure you have all the resources to make registration go as smoothly as possible. When you are ready to register your child, be sure to review the Student Registration Checklist and Requirements before you arrive. The checklist will provide information on the following:

    1. Proof of Residence
    2. School Records
    3. Medical Exams and Records
    4. Immunization
    5. Evidence of Age

    Health Forms

    1. Student Emergency Contact Card (contains consent for school health services and state-mandated screenings)
    2. School Health Entry Exam (Yellow Form)
    3. Authorization for Medication/Treatment
    4. Diabetes Medication Treatment Authorization
    5. Florida Department of Health (FDOH) Dental Sealant Program
    6. Heiken Vision program

    Visiting Our School
    While we welcome you to our school, we also want to ensure the safety and security of our students. Please be prepared to present a valid photo identification upon entrance.

  • REGISTRATION PROCESS 

    • If you are not zoned to Charles W. Flanagan High and would like to apply for reassignment, visit School Choice   

    STEP 2 

    Refer to the Registration Checklist for the required documents. Registration will not begin until all documentation is provided in completion. Please call 754-323-6500 for any.    

    If you share the housing of another person who rents/owns the home (who is not a spouse), you will need to complete a notarized Affidavit of Shared Housing form.

    Schools have the right to verify any information provided by the student and/or the student’s parent(s). A student whose parent(s) submit fraudulent information in an attempt to attend a school to which the student is not assigned shall be immediately withdrawn by the school and must be registered and enrolled in the appropriate boundaried school. Whoever knowingly makes a false statement in writing with intent to mislead a public servant in the performance of his or her official duty, shall be guilty of a misdemeanor of the second degree, punishable by law (F.S. 837.06) or guilty of perjury by false written declaration, a felony of the third degree (F.S. 92.525)

    Must order through the Records Retention Office 754-321-3150 

    Records Retention archives student records of former students who previously attended Broward County Public Schools.  The staff assists schools, departments, outside agencies and former students by searching, retrieving and reproducing archived student records.
