New Student Registration
School registration for new students is in person at our school. Registration is important and we want to make sure you have all the resources to make registration go as smoothly as possible. When you are ready to register your child, be sure to review the Student Registration Checklist and Requirements before you arrive. The checklist will provide information on the following:- Proof of Residence
- School Records
- Medical Exams and Records
- Immunization
- Evidence of Age

The following can be found on the Health Forms website:- Student Emergency Contact Card (contains consent for school health services and state-mandated screenings)
- School Health Entry Exam (Yellow Form)
- Authorization for Medication/Treatment
- Diabetes Medication Treatment Authorization

For the upcoming school year, 2023-2024, the school counseling department has registrations Monday-Thursday from 8:00 a.m. - 10:00 a.m.

Request for Transcript and/or Records
ALL GRADES and Graduates 2017 -Present
- Complete a Transcript Request Form (linked here).
- Pay via the (ESTORE). The transcript fee is $2.00 for in-state schools and $3.00 for out-of-state schools. There is a fee for each transcript printed.
- Bring your ESTORE payment receipt along with your completed Transcript Request Form, to Mrs. Ulfe in the Registrar’s Office in the Guidance Department.
If you wish to email your request to Mrs. Ulfe - your email must include the Transcript Request Form, proof of payment and your driver's license or photo ID all in one email.
Please allow 2 to 4 business days for the transcript to be generated. You will receive an email confirmation when the transcript is ready to be picked up.

REQUEST FOR TRANSCRIPTS must be in writing. We cannot accept phone calls or faxes. Please plan ahead. Due to the high volume, there is a turnaround time of 2-4 business days. This includes proof of student enrollment, graduation verification, immunizations, good student discount forms, social security forms or other government or agency forms.
