    Read and follow the instructions. Student/athletes are not eligible until they are checked off online through https://www.registermyathlete.com/login/

    • Registration for Parents/Guardians CREATE A PARENT/GUARDIAN ACCOUNT:
    • Go to https://www.registermyathlete.com/login/
    • Click Create an Account * You only need ONE account, even if you have children in more than one high school and/or junior high * Do not create another account if you have used Register My Athlete/Aktivate in the past
    • Fill in your personal account information. You will be using the site as a Parent/Guardian.
    • Click Create Account
    • Shortly after creating your account, you will receive an email with a 6-digit Verification Code. Use this code to verify your account when prompted. * Do not close your current tab. You will need to open your email in another tab and find the verification email in your email inbox (it may take a few minutes to appear, so be patient). REGISTER YOUR STUDENT FOR AN ACTIVITY:
    • Login with your email address and password
    • Under the Parents header, click the button labeled Click here to start/complete athlete registrations
    • Click Start/Complete Registrations under "What would you like to do?"
    • Click the New Registration + button in the upper right hand corner
    • Start by clicking the red Select School bar and follow the directions as they appear
    • Continue following the steps presented in the red bars

    Create a Username and password. You only need to register once. If you may be participating in more than 1 sport, then please add all of them when registering. Football, please choose both the 11 man football and Spring Football, unless you are senior. 

    All forms must be 100% complete before trying out or participating in athletics at Monarch. All paperwork must be completed prior to going to practice, tryouts, or off-season workouts.

     

    Checklist for requirements for clearance:

    • Current Physical, completed on the FHSAA Physical Form:
      • FHSAA forms available online on the Aktivate registration site.
      • Please include Page 4 only, unless a followup is required and then also include page 5 of the physical.
      • Physical must be stamped by the doctor's office.
      • DO NOT SUBMIT YOUR IMMUNIZATION RECORDS.

    Top half must be filled out completely.

    Bottom half, must have the doctor's office stamp, the student and parent/guardian signature and dates, and either the box is checked siginfying that there is no relevant medical information needed to provide to further insure the health and well being of the student athlete, or, fill in the areas to let us know what relevant information we should have. This page is used by the athletic trainer to, again, help ensure the student athlete's health.


     Only use page 5 if the student athlete had been rejected by the previous physical evaluation by the doctor's office.

     

    • Current proof of insurance (copy of card) and it is required every year. Must have student's name on it or other proof that the student athlete is covered for the current school year.
      NOTE: Get the 24 hour coverage because athletics is an after school activity. 
      NOTE: Football players, if getting school insurance, be sure to get the coverage for football. Also, this does not cover Spring football or Summer football activieties, more school insureance must be purchased.
    • Birth Certificate Copy.

    • FHSAA Concussion Certificate (watch video, save certificate as pdf file and upload to the Athletic Clearance site) Must have student's first and last name on the certificate. Link available on activate.com site.

    • FHSAA Heat Illness Certificate(watch video, save certificate as pdf file and upload to the Athletic Clearance site) Must have student's first and last name on the certificate.  Link available on activate.com site.  

    • FHSAA Sudden Cardiac Arrest Certificate (watch video, save certificate as pdf file and upload to the Athletic Clearance site) Must have student's first and last name on the certificate.  Link available on activate.com site.

    • FHSAA  Anti Bullying and Hazing Certificate (watch video, save certificate as pdf file and upload to the Athletic Clearance site) Must have student's first and last name on the certificate.  Link available on activate.com site.

    • Parents/Guardians and the student must complete the online registration/signatures forms

    • Must have at least a 2.0 unweighted GPA to be eligible for participation. Beginning of the school year (except incoming 9th graders) and at the beginning of the 2nd semester. If you have completed any online or summer school courses, be sure to let your school counselor know immediately. You will have until the beginning of each semester term to be eligible.

    • If student/athlete has come from a different school, other than middle school, a GA4 form must be completed by the parent/guardian FHSAA.Com site with the form: https://fhsaa.com/documents/2020/4/15/ga04_affidavit_06_2019.pdf  Give the signed form to Mr. Hester.

     


    FHSAA Academic Eligibitlity Information (FHSAA.com)

    Academics

     
    In accordance with Florida Statute 1006.15(3)(a), a student must maintain a 2.0 on a 4.0 scale to be eligible for interscholastic athletic competition. The FHSAA does not have the authority to provide waivers of the minimum GPA requirement.


    A high school student must have a cumulative 2.0 grade point average on a 4.0 unweighted scale, or its equivalent, at the conclusion of each semester to be academically eligible during the next semester

    All high school level courses must be counted towards the student’s cumulative GPA, including those taken before the student entered high school.
     

    A student’s academic eligibility is determined by his/her GPA on the first day of each semester. A student’s academic eligibility cannot change during the semester. Any coursework completed throughout the semester will be calculated into the GPA at the conclusion of the semester.
     
     

    A student cannot be academically eligible if he/she has not attended school and received grades for all courses taken during the previous two consecutive full semesters. For additional academic questions, please contact the Athletic Director at the student’s school of participation.

    FHSAA Requirement for Non-Traditional Students

     
    A non-traditional student is a student who does not physically attend the school at which he/she desires to participate in athletics. Florida Statutes and FHSAA Bylaws permit a non-traditional student to participate based on his/her non-traditional type. These include home education, charter school, district-approved special or alternative school, non-member private school and FLVS Full-Time Public Program students.


    A home education student must be enrolled in the Home Education Program with the school district in which he/she resides.

    Where can a home education student participate?
    A home education student may participate for his/her home-zoned public school, the public school he/she could choose to attend in accordance with the district’s controlled open enrollment procedures (including a charter or lab school), a private school or a home education cooperative.

    What paperwork is required for participation?
    A home education student must complete and submit the EL2, EL3, GA4, EL7 and E7V. If the home education student has chosen a public school through the district’s controlled open enrollment procedures, he/she must also complete the EL14.

    SPORTS

    To participate in a school athletics organization, Home School Education students must complete the Home Education Registration Verification "EL7" form (external link) and send it to the Home School Education office

    Schools/programs may require an original form.

    For correspondence with the BCPS Home School Education Office: 

    If you mail the form, include a self-addressed postage-paid envelope.

    The form will only be signed if your child will be in compliance with an annual evaluation at the time that the sport/music session starts.

    Requests are processed in the order they are received.


     

    A charter school student may only participate in athletics for another school if his/her charter school does NOT offer the sport in which the student wishes to participate.

    Where can a charter school student participate?
    A charter school student may participate for his/her home-zoned public school or the public school he/she could choose to attend in accordance with the district’s controlled open enrollment procedures (including another charter or lab school).

    What paperwork is required for participation?
    A charter school student must complete and submit the EL2, EL3 and GA4. If the charter school student has chosen a public school through the district’s controlled open enrollment procedures, he/she must also complete the EL14.
     

    A special or alternative school student may only participate for another school if his/her special or alternative school is operated by the school district and offers NO athletic program. The special or alternative school must also be authorized by the district superintendent each year. Click here for a list of approved special/alternative schools.

    Where can a special/alternative school student participate?
    A special/alternative school student may participate for his/her home-zoned public school or the public school he/she could choose to attend in accordance with the district’s controlled open enrollment procedures (including a charter or lab school).

    What paperwork is required for participation?
    A special/alternative school student must complete and submit the EL2, EL3 and GA4. If the special/alternative school student has chosen a public school through the district’s controlled open enrollment procedures, he/she must also complete the EL14.
     

    A private school student may only participate for another school if his/her private school is not a member of the FHSAA, does NOT offer the sport in which he/she wishes to participate and has an enrollment of 125 students or fewer in the corresponding grade levels.

    Where can a non-member private school student participate?
    A non-member private school student may participate for his/her home-zoned public school or the public school he/she could choose to attend in accordance with the district’s controlled open enrollment procedures (including a charter or lab school).

    What paperwork is required for participation?
    A non-member private school student must complete and submit the EL2, EL3, GA4, EL12 and E12V. If the non-member private school student has chosen a public school through the district’s controlled open enrollment procedures, he/she must also complete the EL14.
     

    A Florida Virtual School (FLVS) student must be enrolled in the full-time public program. Students enrolled in this program are diploma-seeking. A student enrolled in the FLVS Flex program is considered home education and must be registered as such.

    Where can a FLVS student participate?
    A FLVS student may participate for his/her home-zoned public school or the public school he/she could choose to attend in accordance with the district’s controlled open enrollment procedures (including a charter or lab school).

    What paperwork is required for participation?
    A FLVS student must complete and submit the EL2, EL3 and GA4. If the FLVS student has chosen a public school through the district’s controlled open enrollment procedures, he/she must also complete the EL14.
     

    FHSAA requirements for International Students

     
    An international student is any foreign-born student in the United States that is studying for credit as either a youth exchange student, immigrant, or other international student on a dependent form of a US Visa. Per FHSAA guidelines, International students are offered the opportunity to participate in athletics, should they meet the standards set forth in Policy 17.

    1. J1 Visa: An approved Exchange Agency will write a DS-2019 document for the student’s attendance at a specific school for one year only.
    2. F1 Visa: These students will have an I-20 document written by a CSIET approved school or school district, allowing them to study at that specific school.
    Youth Exchange Students are not permitted to participate as Home Education students.

    What paperwork is required for participation?
    A Youth Exchange Student will be required to provide to the school – proof of age, official transcripts dating back to the student’s 8th year of compulsory education, his/her I-20 (F1) or DS-2019 (J1), an EL2, EL3, and GA4.


    Immigrant students are those that have applied for and have been granted a legal status to remain in the US. Examples: Asylum, DACA, and Permanent Residence requests. These students may or may not have a US Visa.

    Where can an immigrant student student participate?
    A student with a legal status in the US may be eligible to participate at any FHSAA member school.

    What paperwork is required for participation?
    Immigrant students should possess an I-797 – Notice of Action and/or an Employment Authorization Document (EAD). Students with these documents may be eligible for full participation. These students will also need an EL2, EL3, and a GA4.


    Other international students are any non-immigrant students on a dependent form of their parent or guardian’s Visa. Examples: R2, L2, E2.

    Where can an international student participate?
    A student with a dependent Visa may be eligible to participate for any FHSAA member school.

    What paperwork is required for participation?
    These students will possess an I-94 – Arrival/Departure document. This will give the student’s “Admit Until Date”. If this date has expired, the student will not be eligible. A student on a B1/B2 Tourist Visa will be restricted to sub-varsity participation only. In the case of an expired I-94, a student will need to provide a new, unexpired document. If the student is changing their status in the US, they may be considered an immigrant student for athletic participation purposes.

    Students must be on track to graduate and must have 10 of the NCAA-approved core-course credits completed before the start of your seventh semester (senior year), including English, Math, and Science. And must graduate with at least a 2.3 unweighted GPA to be eligible.

    Football School Insurance Plan Requirements:

    Dear Parents: The Broward County School Board requires all 9, 10, 11 and 12th grade tackle football team members to be covered with medical insurance. The School District is not responsible for paying medical bills in the event of a football injury. The following information describes a football accident insurance plan available through Reliance Standard Life Insurance Company. A choice of two benefit levels of coverage is available. You are not required to purchase one of these plans and may purchase your own coverage. However, you must show evidence that you have an insurance policy in effect covering your son before allowing him to be eligible to practice and compete in Broward High School football. This plan may also be used to supplement family insurance that is already in effect through your employer group plan or other insurance policy. This Policy has limitations and exclusions and may not cover 100% of medical charges. The School District is not responsible for payment of any bills not covered by this Policy or your personal insurance policy. We encourage all parents to read the description of coverage and decide whether or not to enroll your child in this Policy.
