    Immunization Requirements for Students Entering the 7th Grade

     

    6th GRADE PARENTS: GET A HEAD START FOR NEXT SCHOOL YEAR!

     

    Pursuant to Florida Statute 1003.22 the immunizations needed:
    Hepatitis B series #1____#2____#3____
    Tetanus-Diphtheria Booster (TD) of TDAP
    MMR vaccine #1____#2____

    Please be sure your child’s vaccines are up to date before the start of their 7th grade school year. If your child needs vaccines, please turn in the new State of Florida 680 Immunization Form before attending 7th grade. Students not in compliance, will not be accepted for entry to school in August. If you have any questions, please contact the school at (754) 322-4000. Drop off the blue form to the Register Office at the Guidance Department to avoid the summer rush.

    Keeping students in Broward County Public Schools safe from infectious diseases is very important. Before entering a Broward Public School, students must have the proper immunizations. For further information, call the Health Education Department at (954) 768-8974. 

    IMMUNIZATION FORMS: All immunizations must be documented on an original Florida Department of Health Form 680 (Florida Certificate of Immunization). For more information, call the District's Health Education Services Department at 754-321-2272 for more information. You may request the form from your child’s physician’s office, visit a public health center.

    According to Chapter 10D.3.088, Florida Administrative Code, prior to entering the seventh grade, all students must have proof of the following immunizations:

    Hepatitis B series (three doses over a six-month period)

    The second dose of measles vaccine (Preferably MMR)Tetanus-diphtheria (Td) booster

    Children entering, attending, or transferring to 8th grade will be required to have completed the Hepatitis B vaccine series, Td booster, and the second dose of Measles vaccine.

    Students who lack proof that they have initiated the series will not be allowed into school until they have proof of beginning the series. (zero tolerance) Students will be allowed to enter school when they submit proof that they have had at least the first Hepatitis B shot, as well as the Td and MMR shots.
