Testing Day Early Sign-Out Form

  • On September 1, 2022, students in the 9th and 10th grade may be released from Everglades High School after the conclusion of the ELA F.A.S.T. exam. 

    The purpose of this form is to obtain parental/guardian authorization for dismissal. 

    The parent/guardian MUST be the one to fill out this form and sign the form below. 

    The form must be filled out completely accurately and MUST include the parent/guardian's driver's license number in order for the student to be allowed off campus.
