Forms for PIP approval to work in schools

All applicants must complete the following:
- The parent/guardian or the PIP must complete the District level Application and Acknowledgement for Private Instructional Personnel (PIPs) Survey.
- Have submitted and had approved a 2021 RBT,or non-RBT PIP, school packet. There are separate packets and processes for PIPs that had an approved application for School Year 2019-20 and those who are making initial application for School Year 2020-21. Please determine and complete the correct forms using the directions in forms below.
- Full packet of Information for principals, parents, and PIP applicants. (current is August 2021 version)
- PIP (RBT) Parent Packet (current is August 2021 version)
- PIP (non RBT) Parent Packet (current is August 2021 version)
- COVID-19 PIP/RBT and Parent(s) acknowledgement form (current is August 2021 version)
Forms from step 2 & 3 above should be submitted to the student's school for processing. The local school administration will verify completion of step 1 upon receipt of completed paperwork.
