Once students are accepted through the magnet office the following items are needed for registration if you are coming from a Private, Charter, or Out of County Public School
COMING FROM PRIVATE SCHOOLS OR OUT OF COUNTY PUBLIC SCHOOLS
*Please bring the following items to registration:
- School Registration Form
- Record Request Form
- Copy of most recent report card (First Semester)
- Birth Certificate (copy)
- Blue or White 680 Health Immunization Form (original)
- Physical Form (original form dated within the year)
- Two (2) Proofs of Address (one from Section A and one from Section B)
Section A
- Current telephone or electric bill in the name of the person registering the child
- Mortgage statement
- Notarized lease agreement with name, address and phone number of leader
- Mortgage commitment
- Home purchase contract including specified closing date with copy of deed to be provided within 60 days of closing date
- Homestead exemption card
Section B
- Automobile insurance
- Current Florida Driver's License/Florida
- Identification card
- Cellular telephone bill
- Credit card statement
- Bank account statement
- United States Postal Service confirmation of address change request
- Declaration of Domicile form from the County Records Department
*SCHEDULES WILL NOT BE FINALIZED UNTIL RECORDS ARE RECEIVED FROM THE PREVIOUS SCHOOL

New Student Registration
School registration for new students is in person at our school. Registration is important and we want to make sure you have all the resources to make registration go as smoothly as possible. When you are ready to register your child, be sure to review the Student Registration Checklist and Requirements before you arrive. The checklist will provide information on the following:- Proof of Residence
- School Records
- Medical Exams and Records
- Immunization
- Evidence of Age
Health Forms
The following can be found on the Health Forms website:- Student Emergency Contact Card (contains consent for school health services and state-mandated screenings)
- School Health Entry Exam (Yellow Form)
- Authorization for Medication/Treatment
- Diabetes Medication Treatment Authorization
- Florida Department of Health (FDOH) Dental Sealant Program
- Heiken Vision program
