  • In order to receive a Certificate of Completion for this training, you must demonstrate that you are able to use the information gained in a realistic manner.  At some time in your career, chances are great that you will actually encounter an abuse situation and need to make a hotline report.  Right now, you are being given some Case Studies to read.  Choose one of these case studies and use it as the basis for completing a practice child abuse report and a case example requirement form.  The child abuse designee at your school must sign off at the bottom of the second page of your case example requirement form.

    For your practice child abuse report, I am looking to see that you have filled in all sections of the form. The point of this exercise is that investigators may not be successful if you do not give complete and accurate information to the hotline.

    Place the completed Case Example Requirement Form, completed Child Abuse Report Form (4002B), and the completed Evaluation Form in a PONY envelope and send them to Yva Dieudonne at:

    NOTE: Remember to complete the Online Test and submit it electronically.

    Your post- test score will be shown when you submit.

    You must receive a score of at least 80 percent correct on the Test to complete the child abuse training. Only two re-tests are permitted, so review before you take another test.

    Make copies of the forms that you submit, in case they are lost in transit.  When your correctly completed materials are received and you have passed the Post-Test, a Certificate of Completion will be returned to you.  Place this certificate in your professional portfolio, along with your case example.  If you are a new employee this year, give a copy of your certificate to your administrator.
