• Certificate Issuance Request Contracted Community Agencies. Submit this form when hiring a teacher who does not currently hold a valid Florida Educator Certificate. NOTE: A Statement of Status of Eligibility is not the same as an Educator Certificate.

Prior to submitting the Certificate Issuance Request to Talent Acquisition & Operations - Certification, ensure the contracted teacher has been fingerprinted using the information below:

• Fingerprint processing and background checks for community agency instructional applicants will be performed by FieldPrint, Inc.  Find out more detailed instructions for using FieldPrint.
• The hiring location must provide the applicant with the FieldPrint Code in advance of the applicant registering to be fingerprinted through FieldPrint.
